How fees work
Replacing teeth is done in steps, so the fee is too. Here is how paying works, in the order patients ask.
Insurance we work with
We send claims to dental plans for you: in network with some PPO plans, and out of network with others. Tell us your plan before your first visit and we'll say which it is. Dental plans often pay only part of implant care, or none of it, and most have a yearly limit. We are not enrolled with Medicaid or CHIP.

If you pay yourself
Without insurance, or for care a plan doesn't cover, you pay by the phase as treatment goes on. Your estimate lists every phase and its fee before you begin, and you decide each phase as you reach it. Financing is offered by third-party lenders. Approval and terms are set by the lender.

Receipts for reimbursement
After each visit you get an itemized statement with the procedure codes. If your plan reimburses out-of-network care, you can send it in. Whether it pays, and how much, is the plan's decision.

An estimate in advance
You can ask for a written estimate before any treatment. If you're paying without insurance, we give you a written estimate in advance, when you schedule or whenever you ask. It lists each phase, the visits and the fee for each, so you can see what comes next.

Checking your benefits
Before your first visit, Anabel Rosas can look up what your plan lists for implants, crowns, bridges and dentures and tell you what it says. A plan's answer before treatment is an estimate, not a promise to pay.

What the first visit includes
About an hour: a talk about what you want, an exam, a 3D scan if Dr. Durham needs one, and the first outline of a plan. Bring a photo ID, your plan card if you have one, and any X-rays. Health forms are in our private patient portal.

What an estimate lists
Nothing starts until you have read it and said yes.
The phases
Surgery, healing, and the final teeth, each with its own line.
The visits
How many we expect, and what happens at each one.
The lab work
The crowns, bridges or dentures a dental lab makes for you.
The follow-up
Checkups after the final teeth, so you know what comes after.
Ask about a fee before you come
Anabel can tell you how an estimate works over the phone, and what to bring.
